Mr. Sean Young is a charge nurse on a medical unit that includes telemetry-monitored rooms. The nursing staff includes registered nurses (RNs), graduate registered nurses (GNs), licensed practical nurses (LPNs), and unlicensed assistive personnel (UAPs).

At the start of the shift, Sean prioritizes care and responds to a call about a client being admitted from the ED. Sean responds to several situations, assigning care to the RNs and GNs, delegating tasks to the UAP, and collaborating with members of the interdisciplinary team (IDT).

Sean responds to an LPN who requests a change in the assignment. Client care assignments are changed when a staff nurse becomes ill and leaves.

In supervising the UAP, the RN observes incorrect client care, and both the RN and charge nurse respond to the situation. Sean selects a client for transfer from the unit and makes arrangements for the client’s safe transfer. When a new client is admitted, the primary nurse serves as a mentor to the GN and supervises the care provided by the GN.

Staff members attend an in-service program on the use of new insulin pens. Sean evaluates the effectiveness of the training and assists an LPN who has difficulty manipulating the pens. Later, the nurses report that patients are not using the pens successfully. A quality improvement plan is developed. Sean helps develop the goal for the plan, and data is collected. A recommendation will be made with input from the IDT.
