DEVELOPING A PROGRAM PLAN OUTLINE
INSTRUCTIONS
After reviewing the assigned readings for the course to date, you will:
1. Identify your priority community diagnosis.
2. Use thediagnosis selected to serve as the basis for developing your community intervention plan.
3. Reference Box 12.2 in your text as a guide and develop an outline that includes the following content areas:
• Executive summary
• Introduction
• Goals
• Objectives
• Program
• Intervention
• Resources and constraints
• Budget
• Appendices
4. Be very detailed in the development of this outline. Refer to resources that are provided and that are related to the content outline development. You will use this assignment as a frame of reference for the development of your final program plan document in NURS 633.
5. Support your work with evidence from 3–5 scholarly sources in addition to the course textbook. References must have been published within the past 5 years. The outline must be a full sentence, alpha-numeric outline that is 2–3 pages (excluding the title page and reference page).

Major points are supported with evidence from scholarly sources. At least 3 scholarly sources are required in addition to the course textbook and the Bible. References must have been published within the past 5 years. 6 to 7 points
Provides a somewhat detailed discussion of the community diagnoses. Be very specific and identify why this diagnosis was prioritized over the others.

The following applies to this and each of the following content-related categories of this rubric:
Major points are somewhat supported with evidence from scholarly sources and/or there is a minimal discrepancy in the required number and/or type of references utilized within the paper. References must have been published within the past 5 years.

Provides a limited discussion of the community diagnoses. The discussion lacks specificity and does not clearly identify why the diagnosis was prioritized over the others.
