For this assignment, you will use your proposed PICO question and variable table from W3 Assignment 1: Flushing out your Evidence Based Practice Project to develop a 3-4 page paper.
For this paper include the following:
• Provide an overall introduction to the paper.
• Describe what the social determinants of health (SDOH) are, and why they are important to address.
• Describe the population of the organization in which you would pull a sample to complete your project and collect your data. Utilize an organization, county, city, region or state level community health assessment for specific population data.
• Discuss to what extent 3 different types of SDOH impact your potential population.
• Discuss how a DNP prepared nurse can change organizational processes to incorporate SDOH considerations in the policy/procedure development.
