For this week’s discussion, use the readings and resources to formulate responses to the following questions:
• Define patient-centered care, family centered care, and patient/family centered care.
• What is your definition of health?
• Tell us about a time where you had a challenging patient and/or family caregiving situation where they might have been considered “non” compliant.
o What do you think their definition of health was?
o What social determinants of health may have influenced the patient/family’s beliefs?
• How would a DNP prepared nurse bridge the gap between you and the patient definition of health and come to an agreed upon plan of care?
• How would you as a DNP prepared nurse mobilize leadership support for change?
