A windshield survey is a valuable assessment tool that can assist the community-based nurse in performing many facets of his/her job. This unique tool is used to collect objective data that can be used to provide a rapid overview of potential health and wellness issues faced by the city or town in which they work. It is a scientific method commonly utilized to gather data within communities. It requires you to travel throughout your identified community and gather data that you can SEE, that you can HEAR, that you can SMELL, and can perhaps TOUCH. For the Windshield survey, you should imagine you are a community based nurse assigned to conduct this assessment in a community you know nothing about, and will be visiting for the first time. You may select any community you wish to focus on throughout the semester.

· Please note that in a Windshield Survey the data collected is OBJECTIVE rather than subjective.

· You must not add information that you possess because you have previous knowledge of the identified community

· It must not include your perspective or beliefs about what you feel the community contains, or its strengths/weaknesses from your perspective.

· It must not be data you collected from looking at a community website or reading the community newspaper.

· It is data gathered by walking or driving around the community and observing it.

· The windshield survey is frequently used as a “first pass tool” for defining key areas in need of primary, secondary or tertiary intervention.

· It is one area of data collection you will use to support your Community Assessment paper.

· Conduct a windshield survey driving around the neighborhood. This survey will count for 2 hours of on your Log of Hours. If possible, have someone else drive while you take notes or plan to stop frequently to write down your observations.

· Be sure to review Rubric for specifics of APA formating of paper. There are no references required, but proper formating is required: title page, Level 1 headings to match instructions.

Record objective data on the ten components of your Windshield Survey (listed in instructions attached below) and write one or two well developed paragraphs (not an outline) on each of the 10 components. If it is not possible to gather data on any of the components, write a brief explanation of the reason.
